Reporting Injuries, Incidents and Safety Concerns

As a reminder, all members of the university community are encouraged to report safety concerns, whether on campus or encountered while involved in university affiliated activities, to Campus Safety and Emergency Management by emailing safety@uthsc.edu. Employee and student injuries must also be reported. Employee injuries must be reported by calling Corvel AND using the online Incident Report Form (select On-The-Job Injury) to notify UT Risk Management. Student injuries must be reported to UT Risk Management using the online Incident Report Form (select General Liability).

The procedure for reporting incidents is accessible in the Safety Manual (GS5400 – Reporting Injuries, Incidents and Property Damage) accessible through the university website. Also addressed are procedures for reporting property damage and auto accidents that occur while on university business.
