If qualified, employees can buy additional voluntary term life insurance for themselves, their spouses and their children. Enrollment is not automatic. An employee must apply for this coverage and complete an Evidence of Insurability Form.
This insurance is in addition to the employee basic term life and basic accidental death and dismemberment insurance. You may elect any amount, in the increments of $5,000 up to a maximum of the lesser of seven times your annual base salary or $500,000.
The employee’s spouse may apply for enrollment in this coverage even if the employee does not enroll. The amount available on a spouse is based on the age of the spouse when application is made, as follows: Less than age 55: You can enroll for $5,000, $10,000, $15,000, $20,000, $25,000 or $30,000.
Age 55 or older: You can enroll for $5,000; $10,000 or $15,000.
The guaranteed issue amount is $5,000.
A child term rider may be added to the employee’s or spouse’s certificate, but not to both.
An employee or spouse who enrolls may add a child term rider in the amount of $5,000 or $10,000. Only the employee or spouse may have a child term rider attached to his/her certificate.
