Quest Diagnostics will be at the Student Alumni Center Wednesday, August 26, from 8:00 am to 3:00 pm, to complete Biometric Screening for employees and spouses.
Returning employees (who already have a username and password) can schedule a screening at my.questforhealth.com.
New employees (who have never completed a screening) must first register with Sharecare at https://stateoftn.sharecare.com/ and log in. Navigate to Achieve and click on the Programs tile. Select Health Screening to go to the Quest site (create username and password) and select your screening option.
Once on the Quest website, you may either download a physician screening form or sign up for an onsite screening.

Take the RealAge Test to look at your habits and health risks to calculate the true age of your body and to personalize your Sharecare profile. To get started, go to stateoftn.sharecare.com.
Once an employee and/or spouse completes Active Health’s online health assessment, they can each earn up to $250 a year by completing certain wellness activities (if eligible). Each participant will be able to earn a maximum of $250 per person ($500 annual maximum).
