To err is human. The FDA receives more than 100,000 reports related to medication errors annually, and an estimated 7000 to 9000 deaths each year are attributed to these errors. Medication errors can occur at any point in the complex medication-use process and continue to occur despite prevention strategies.  Creating a culture of patient safety involves advocating for patients, learning from past errors, and identifying practices professionals can employ to prevent and mitigate errors
