Take the worry out of getting your required biometric health screening.

The Partnership PPO health insurance plan offers great benefits, and in order to keep them, you and your covered spouse must complete annual requirements. One requirement is to get a biometric health screening.  Luckily, it’s easy to do. You can complete it at an onsite location or your doctor can do it.

The deadline is July 15, 2014.

1.  Get your biometric health screening at an onsite work screening.  Use the online scheduler to set up an appointment:  https://my.onsitehd.com/restricted/signup/tn

****UTHSC ONSITE WORK SCREENINGS WILL BE HELD JUNE 18-19, 2014 from 7:00 am – 1:30 pm in the Student Alumni Center (SAC) Dining Hall, 800 Madison Avenue.

2. Complete your screening (annual physical) at your physician’s office. Download and print a form https://my.onsitehd.com/restricted/signup/tn which must be completed by a health care provider. Either you or your doctor can send your completed and signed form to Onsite Health Diagnostics (OHD).

Healthways will accept screening results from a doctor’s visit between July 16, 2013, and July 15, 2014. If you’ve already completed your annual physical, just download your form and ask your doctor to fill in the results.

To receive your screening form in the mail, call 1.888.741.3390 and select option 1 to order your physician screening form from the automated system.
